The board certified medical staff at the Arthritis, Neck and Back Pain Center are proud to provide individualized, empathetic care to all patients.
The Arthritis, Neck and Back Pain Center provides diagnosis and treatment for a variety of neck, back, joint, pain and autoimmune problems including: Arthritis; Back Pain; Cervicalgia; Fibromyalgia; Gout; Knee Pain; Lupus; Myofascial pain syndrome; Neck Pain; Neuropathy; Osteoarthritis; Osteoporosis; Psoriatic Arthritis; Sjogren's syndrome; Spondylitis.

Release of Information
If you are a current or former patient, you may retrieve a copy of your healthcare record by filling out and signing a HIPAA-compliant release of information form and submitting it to us via mail, fax or e-mail. If you are a legal firm, insurance company or other third party, please contact our office for information on records retrieval. A fee may apply for records retrieval, copying, mailing and/or notarizing of affidavits.

Insurance Plans
We are an in-network provider for payors including:

Please confirm coverage and eligibility with your insurance company before your appointment.
If your insurance plan requires a referral, it must be received by our office before your appointment can be scheduled.

Payment Policies
Any applicable co-pays are due at the time services are rendered.
If you have received a bill from us for a balance owed, payment may be made by check, or by credit card or cash at our office.
