Release of Information
If you are a former patient, you may retrieve a copy of your healthcare record by filling out and signing a HIPAA-compliant release of information form (below) and submitting it to us via mail, fax or email. If you are a legal firm, insurance company or other third party, please contact us for information on records retrieval. A fee may apply for records retrieval, copying, mailing and/or notarizing of affidavits.
